PACE KC Program and Adult Wellness Center

PACE (Program of All-Inclusive Care for the Elderly) is a healthcare program that provides a full range of health and wellness services to allow older adults to age gracefully in their home communities.

Swope Health has launched a capital campaign for the development of this program in Jackson County, to be called PACE KC, and for a new state-of-the-art Adult Wellness Center where many PACE services would be provided to qualified Jackson County residents.

A long-term care option unlike any other in our area, PACE KC will be designed to follow the model successfully used at 140 organizations nationally to offer the entire continuum of care and services to individuals age 55 and older, with a proactive, hands-on approach to personalized healthcare.

PACE emphasizes comprehensive, coordinated and preventive care that treats the whole person – addressing physical, emotional and psychological needs. A team of providers works together across many disciplines to help PACE members achieve and maintain their health and wellness goals.

Eligibility for PACE KC would require members to be Jackson County residents who are 55 or older, certified by MO HealthNet, and able to live safely in the community with PACE support at the time of enrollment. The PACE model offers the following:

  • Comprehensive healthcare – from primary care to specialized care and every level in between
  • A unique health insurance plan that supports the goal of healthy independent living
  • A team of personal health navigators to provide guidance at every step of aging in place
  • Door-through-door transportation to and from all care-related visits and activities
  • Prescription drugs and prescribed over-the-counter medications, including convenient, reliable at-home delivery with personalized instructions
  • Home care services to support health and safety in the home

Located adjacent to Swope Health Central at 4141 Dr. Martin Luther King Jr. Blvd., the PACE KC Adult Wellness Center will be the planned home of the PACE KC interdisciplinary care team and the central hub for most PACE KC services.

The 32,000-square-foot, two-story building will be welcoming and comfortable, with opportunities for socialization, fitness classes, meals and companionship. Along with clinical spaces to provide primary care, medical services and other therapies, plans include a day center, rehabilitation gym, computer lab, library, arts and crafts room, bathing suite, low-stimulation memory care area, outdoor recreation space and meeting rooms. At full capacity, PACE KC would provide 90 high-quality jobs.

In July 2021, the Sunderland Foundation announced a grant of $1.5 million to support the project. We hope to break ground on the PACE KC Adult Wellness Center in late 2021 or early 2022, with planned opening in 2023 for this key addition to neighborhood development.
